Regulator warns Air India over delayed emergency equipment checks: Report
According to the DGCA report, spot checks carried out in May found that three Air India Airbus aircraft were flown even though scheduled inspections on the escape slides – considered “critical emergency equipment” – were overdue.

INDIA’s aviation regulator has warned Air India for violating safety rules after three of its Airbus aircraft operated flights without undergoing mandatory checks on emergency escape slides, according to official documents reviewed by Reuters.
The Directorate General of Civil Aviation (DGCA) issued warning notices and a detailed investigation report highlighting the breach. These documents were sent days before the recent crash of an Air India Boeing 787-8, in which all but one of the 242 people onboard were killed. The Airbus incidents are unrelated to that crash.
According to the DGCA report, spot checks carried out in May found that three Air India Airbus aircraft were flown even though scheduled inspections on the escape slides – considered “critical emergency equipment” – were overdue.
One Airbus A320 was flown for more than a month before the check was completed on 15 May. AirNav Radar data showed that the aircraft operated international flights during that period, including to Dubai, Riyadh and Jeddah.
In another case, an Airbus A319 used on domestic routes had slide checks overdue by over three months. A third case involved a plane where the inspection was two days late.
“The above cases indicate that aircraft were operated with expired or unverified emergency equipment, which is a violation of standard airworthiness and safety requirements,” the DGCA report stated.
The report also said Air India failed to respond on time to DGCA’s queries on the issue, which “further evidenc[ed] weak procedural control and oversight.”
Air India, which was acquired by the Tata Group from the government in 2022, said in a statement that it was “accelerating” verification of all maintenance records, including escape slide checks, and expected to finish the process soon.
In one case, Air India said the issue came to light after an AI Engineering Services engineer “inadvertently deployed an escape slide during maintenance.”
The DGCA and Airbus did not respond to requests for comment from Reuters.
Vibhuti Singh, a former legal expert at India’s Aircraft Accident Investigation Bureau, said checks on escape slides are “a very serious issue. In case of accident, if they don’t open, it can lead to serious injuries.”
The DGCA report said aircraft that miss mandatory inspections have their airworthiness certificates “deemed suspended.”
The warning notices and report were issued by Animesh Garg, deputy director of airworthiness, and were addressed to Air India CEO Campbell Wilson, as well as the airline’s continuing airworthiness manager, quality manager and head of planning.
An Indian aviation lawyer told Reuters such breaches often lead to monetary and civil penalties for both the airline and individual executives.
Wilson said in a 2023 interview with Reuters that global parts shortages were affecting most airlines, but the problem was “more acute” for Air India, whose “product is obviously a lot more dated,” with many aircraft not updated since 2010-2011.
‘Systemic control failure’
The DGCA report said some Air India aircraft inspected by officials also had outdated registration paperwork. Air India told Reuters that all but one of its aircraft met the required standards and said this “poses no impact” to safety.
The report criticised the airline’s internal oversight systems, stating: “Despite prior notifications and identified deficiencies, the organisation’s internal quality and planning departments failed to implement effective corrective action, indicating systemic control failure.”
India’s junior aviation minister told parliament in February that airlines were fined or warned in 23 cases over safety violations last year. Of these, 12 involved Air India and Air India Express. One case involved “unauthorised entry into cockpit”, and the biggest fine – $127,000 – was imposed on Air India for “insufficient oxygen on board” during a San Francisco-bound flight.
Air India’s Chairman N Chandrasekaran addressed employees on Monday, calling on staff to remain focused amid criticism following last week’s crash and said the incident should serve as a catalyst to build a safer airline.
Muttahida Qaumi Movement (MQM) founder Altaf Hussain has been hospitalised in London after falling seriously ill, according to a party official.
Hussain, 71, was admitted to a hospital on Thursday due to a severe illness, where doctors carried out various tests, Mustafa Azizabadi, Convener of MQM’s Central Coordination Committee, said on social media.
"The founder and leader of MQM, Altaf Hussain, has been admitted to a hospital in London due to severe illness, where various tests have been conducted on him,” Azizabadi said.
“Doctors have conducted various tests and are focusing their attention on his treatment,” he added in a video message, as quoted by Dawn.
Hussain has been living in London since 1992 and holds British citizenship, the report said.
He often delivers political speeches to his supporters in Karachi through social media platforms.
Hussain founded the party in 1984 under the name Muhajir Qaumi Movement to represent the Urdu-speaking community that migrated from India to Pakistan during the 1947 partition.
Born on September 17, 1953 in Karachi, he started his political career in 1978 by founding the All-Pakistan Muhajir Students Organisation (APMSO).
In the 1988 general elections, MQM won a majority in Sindh’s urban areas and became the third-largest party in the country, Dawn reported.
Hussain has remained in self-imposed exile since the early 1990s after the government launched an operation in Karachi at the time.

FOUR weeks before an Air India Boeing 787-8 crashed after takeoff from Ahmedabad, media reports cited a safety notice issued by the UK's Civil Aviation Authority (CAA) highlighting potential issues with fuel control switches on Boeing aircraft.
The CAA has now clarified that the safety notice in question — Safety Notice Number SN-2015/005 — was originally issued in 2015. The document was updated on 15 May 2025 only to change the contact email address. This routine administrative update caused the document to appear on the CAA website as if it were newly issued.
A CAA spokesperson said: “Whilst the Air India accident investigation is ongoing, we do not have any technical concern with regards to Boeing products and we do not require any additional actions from UK operators.”
According to India Today, the notice was dated May 15 and instructed operators of Boeing models including the 787 Dreamliner to assess a US Federal Aviation Administration (FAA) Airworthiness Directive concerning fuel shutoff valve actuators. These valves are safety devices designed to stop fuel flow to engines.
"The FAA has issued an Airworthiness Directive (AD) addressing a potential unsafe condition affecting fuel shutoff valves installed on the following Boeing aircraft: B737, B757, B767, B777, B787," the CAA notice stated, according to the report.
The CAA had ordered daily checks and possible testing, inspection or replacement of affected parts. The issue drew renewed focus after India's Aircraft Accident Investigation Bureau noted in its preliminary report that the aircraft’s fuel control switches unexpectedly moved to “CUTOFF” after liftoff, shutting down both engines.
The FAA later said the switch design posed no safety risk, and Boeing issued similar guidance. Air India replaced the Throttle Control Module in 2019 and 2023 but did not inspect the switch locking mechanism, saying the 2018 FAA advisory was not mandatory. CEO Campbell Wilson said no cause had been identified yet.

A PRELIMINARY report into last month's Air India plane crash that killed 260 people has raised further questions, and the investigation is still ongoing, Air India CEO Campbell Wilson said in a memo to staff on Monday.
The initial report, released by India’s Aircraft Accident Investigation Bureau (AAIB) on Saturday, pointed to confusion in the cockpit shortly before the Boeing 787 Dreamliner crashed.
In a staff memo reviewed by Reuters, Wilson wrote: “The report had triggered a new round of speculation in the media ... Unsurprisingly, it provided both greater clarity and opened additional questions.”
He added: “The preliminary report identified no cause nor made any recommendations, so I urge everyone to avoid drawing premature conclusions as the investigation is far from over.”
No mechanical faults found
The memo noted that the report found no mechanical or maintenance faults, and confirmed all required maintenance had been completed.
The Boeing 787 Dreamliner was flying from Ahmedabad to London when it began losing thrust shortly after takeoff. Of the 242 people on board, all but one died. Nineteen people on the ground were also killed.
According to the AAIB report, one pilot asked the other in the final moments of the flight why he had cut off the fuel.
“The other pilot responded that he did not do so,” the report said. It noted that the fuel cutoff switches for engine two flipped almost simultaneously, but did not specify how.
The report did not suggest any immediate action for Boeing or GE, which supplied the aircraft’s engines.
Pilot body rejects early conclusions
ALPA India, which represents Indian pilots at the International Federation of Air Line Pilots’ Associations, rejected any presumption of pilot error and called for a “fair, fact-based inquiry”.
Campbell Wilson added in his memo: “The pilots had passed their mandatory pre-flight breathalyser and there were no observations pertaining to their medical status.”
The flight’s commanding pilot was 56-year-old Sumeet Sabharwal, who had logged 15,638 total flying hours and was also an Air India instructor, according to the Indian government. His co-pilot, Clive Kunder, 32, had 3,403 hours of experience.
Air India faces broader scrutiny
Following the crash, Air India has come under increased scrutiny on multiple fronts.
On July 4, the European Union Aviation Safety Agency said it would investigate Air India Express, the airline’s budget unit, after a Reuters report revealed it had failed to promptly replace engine parts on an Airbus A320 as mandated and falsified records to indicate compliance.

FUEL control switches to both engines of the Air India flight that crashed shortly after takeoff were moved from the "run" to the "cutoff" position seconds before the crash, according to a preliminary investigation report released early Saturday.
The report, issued by the Aircraft Accident Investigation Bureau (AAIB), did not draw conclusions or assign blame for the June 12 crash, but stated that one pilot was heard asking the other why he had cut off fuel. The second pilot replied that he had not.
The Boeing 787-8 Dreamliner was on its way from Ahmedabad in western India to London when it crashed, killing 260 people. Among the dead were 241 of the 242 people on board and 19 people on the ground.
Switches moved one after another
The 15-page report said that after the aircraft reached its top recorded speed, "the Engine 1 and Engine 2 fuel cutoff switches transitioned from RUN to CUTOFF position one after another with a time gap of 01 sec".
"In the cockpit voice recording, one of the pilots is heard asking the other why did he cutoff. The other pilot responded that he did not do so," the report stated.
The aircraft quickly lost altitude following the switch movement.
The switches then returned to the "RUN" position, and the engines appeared to regain power. However, "one of the pilots transmitted 'MAYDAY MAYDAY MAYDAY'," the report said.
Air traffic controllers asked the pilots what was wrong, but soon observed the crash and alerted emergency responders.
Investigation focused on fuel switches
Earlier this week, specialist website The Air Current reported that the probe had "narrowed its focus to the movement of the engine fuel switches", while also stating that a complete analysis may take several months or more. It added that the focus of the investigation could still change.
According to the AAIB report, the US Federal Aviation Administration had issued an information bulletin in 2018 about "the potential disengagement of the fuel control switch locking feature".
The issue was not considered an "unsafe condition" requiring a more serious directive, and Air India told investigators it had not conducted the suggested inspections as they were "advisory and not mandatory".
Air India was found to be compliant with all airworthiness directives and alert service bulletins on the aircraft, the report noted.
The bureau said there were "no recommended actions to B787-8 and/or GE GEnx-1B engine operators and manufacturers", indicating that there were no immediate technical concerns with either the aircraft or the engines.
The AAIB said the investigation is ongoing and that further evidence and information have been "sought from the stakeholders".
Boeing said in a statement that it would "continue to support the investigation and our customer", adding, "our thoughts remain" with those affected by the crash.
Air India stated that it was "working closely with stakeholders, including regulators".
"We continue to fully cooperate with the AAIB and other authorities as their investigation progresses," the airline said in a statement posted on X.
Passenger details and international assistance
The UN's International Civil Aviation Organization (ICAO) requires that a preliminary report be submitted within 30 days of an accident by the country leading the investigation.
Investigators from the United States and the United Kingdom have also joined the probe.
The aircraft was carrying 230 passengers — including 169 Indian nationals, 53 British citizens, seven Portuguese nationals and one Canadian — along with 12 crew members.
Dozens of people on the ground were also injured.
One British passenger survived and was seen walking out of the wreckage. That person has since been discharged from hospital.
Initial figures released by health officials in Gujarat put the death toll at 279, but forensic experts later revised the number after identifying multiple scattered and severely burnt remains.

A Chinese research vessel was detected operating in the Bay of Bengal near Indian waters while attempting to conceal its presence by disabling its Automatic Identification System (AIS), according to a report by The Economic Times, citing French maritime intelligence firm Unseenlabs.
The French company conducted a 16-day satellite-based survey tracking ships through radio frequency emissions. It monitored 1,897 vessels, with 9.6 per cent showing no AIS activity, indicating attempts to avoid detection. The survey raised concerns amid increased Chinese activity in the region.
One Chinese research ship drew attention due to its repeated presence in the Indian Ocean and proximity to areas of recent naval operations. “While not broadcasting AIS, its RF signature was consistent and traceable, enabling our systems to monitor its movement over several days … We suspect that this prominent Chinese research vessel was likely operating with strategic intent,” the report quoted Unseenlabs.
The company said the vessel was likely engaged in seafloor mapping, acoustic environment analysis, and identifying submarine transit corridors—activities that can support surveillance and anti-submarine warfare operations.
The Indian Navy and Coast Guard have consistently reported Chinese research vessel presence. In this instance, the vessel was in international waters, 120 nautical miles from Indian military zones.
